Synappz Mobile Health’s is the developer of the Clinicards platform. With this platform, healthcare providers can use digital ‘cards’ for easy monitoring of patients’ clinical variables in a home setting, to inform patients and put together care plans. The patient receives a customised app with which they receive coaching. Clinicards has been developed together with healthcare professionals and is now being used by a number of Dutch hospitals in areas such as neonatology, pregnancy, pulmonary embolism, rehabilitation and post-operative surgery.
